Safety Mishap and Accident Reporting

If you are involved in an accident or mishap, you must immediately contact the Squadron Commander, Capt Jose Avila at (732) 585-9059 or (732) 583-5695.  If unavailable, immediately contact the New Jersey Wing Commander,(Col Bob McCabe at (856) 912-5329, and the New Wing Safety Officer, LtCol. Kevin Webster at 973-903-1939.

In the event that you cannot contact any of the above, you must contact the New Jersey Wing Vice-Commander, Lt Col Robert Jennings at (856) 266-0418.

Copies of all CAP Form 78s needs to be submitted immediately after the safety mishap or accident, or within 48 hours to Lt Col Webster's fax which is (815) 550-0629.  Refer to the New Jersey Wing and NER Safety Supplements for additional information
